Book an appointment online... Online appointment bookings require a minimum of one week lead time. For shorter times, please call our office. To book online follow these steps:

• Click on the submit button to send your request
• You will get a confirmation screen indicating your request has been sent to us
• We will contact you directly by email, FAX or phone to confirm your appointment request
